New Season Treatment Center is excited to announce an important advancement in our admissions process; our consent forms are now available digitally through the Digital Pre-Admission Form. This online tool enhances both the patient experience and team efficiency, creating a smoother, faster, and more patient-centered start to care.

How It Works: A Step-by-Step Experience
The Digital Pre-Admission Form was designed with simplicity and accessibility in mind. Here’s how it works from start to finish:
1. Obtaining the Link
Patients can receive the form via email from our 24/7 Call Center at 1-877-284-7074. The form must be completed by the patient themselves.
2. Starting the Form
Patients can open the link on a computer or mobile device to begin. The Consent & Privacy section must be completed within 72 hours of intake to remain valid.
3. Reading the Introduction
An introductory section explains what to expect, and patients who need help can email telehealth@cmglp.com for support.
4. Completing Required Information
Required fields are clearly marked with an asterisk (*), ensuring no critical details are missed. Supporting documents can also be uploaded securely.
5. Selecting a Center
Patients select their preferred treatment center — which auto-populates for accuracy — ensuring a seamless intake experience.
6. Submitting the Form
Once completed, the form is automatically sent to the selected treatment center’s inbox and securely labeled with the patient’s last name and submission date for easy tracking.
“Automation doesn’t replace our care; it enhances it,” Oliva said. “The digital form helps us prepare for each patient before they arrive, making intake more personal and efficient.”

- Validate Online Submissions — Verify information for accuracy (e.g., phone numbers, emergency contacts).
- In-Person Orientation — Must still take place on-site to review regulations, clinic operations, and counseling expectations.
- Provide the Welcome Packet — Every patient should receive the packet, which includes the Patient Handbook and key resources for their recovery journey.
